The Peeper appears as a wandering eye. It is in fact Isaac's left eye which has been taken out of his socket leaving a bloody empty hole behind:

Strategy Guide/Tips

• Even though your left eye socket becomes red, you do not shoot red tears. It's purely cosmetic.
• The Peeper leaves a small trail of blood which does no damage, purely cosmetic
• The Peeper is kinda slow compared to other familiars.
• The Peeper will also block enemy shots if it's in the way.
