Simon the Necromancer DLC4
Simon the Necromancer is a mage in Tiny Tina's Assault on Dragon Keep DLC.
He gives you the quest My Dead Brother where he wants you to find the corpse of his brother in the Lair of Infinite Agony so you can kill him again. He puts a curse on you, that while very look, will kill you in a week (not really but story).
At the end of the quest you can side with him or his brother Edgar.
Siding with Simon will get you a Blue Shotgun.
