As Isaac makes his way from the shipwreck to the Mining Facility in the beginning of Act 2, he climbs over a bridge.
The ground below him gives way, and he ends up falling through a forklift, dangling by the door over the snowy precipice below.
A Quick-Time Event then occurs, sending the player scrambling through the forklift and safely out the other side.
Completing the QTE[edit]
The player must first mash on the A or X button on Xbox 360 and Playstation 3, respectively. This will help Isaac maintain his hold on the door.
Then, the player must push the Left Stick forward to climb through the forklift and over to the other side.
