There is 1 Dead Ghost to be found in Hall of Guardians. They can be collected any time. Each Ghost can only be collected once per account.

Head into the Hall of Guardians, down into where the Vanguard mentors are:
Once inside, turn to the right. Look on the table that is just inside the doorway. You will see the Dead Ghost sitting right on it.
