Damaged items have to be repaired to restore efficiency in The Witcher 3: Wild Hunt
"One of your items is in bad shape. Repair it - otherwise it won't work as well."
To repair a weapon, you need to find a blacksmith or armorer, such as Willis in White Orchard
Open the shop panel and choose the "Repair" tab to access the repair menu.
