If there is no achievement even though you have been everywhere and not a single area is blacked out, then you probably missed a zone. The most common place people missed is the place is where you get beware of the clap challenge, there's a sacrifice hanging from a rope and you hit switch on the left to call a huge claptrap to burn it alive. When you get to Scylla's Grove, check your map in the lower left part, there should be a very small area that is not uncovered at an elbow joint. When you go to this area to look around, you want to look at the walls and find a small cave. See map below for help finding the location, red "X" marks the spot:
